Parent Enrollment Survey:
All parents asked to submit a Parent Enrollment Survey to select either full in-person or virtual learning for each student in their household. Parents only need to complete one survey per household, but must list each student separately within the survey. Parents must also indicate whether a student selecting in-person instruction will utilize AM and/or PM transportation. The survey is due no later than noon on Monday, March 22nd.
Frequently Asked Questions:
The district has developed a Frequently Asked Questions document that provides information on the most common topics related to the transition to full in-person learning. This document will be updated on a regular basis as more information becomes available.
As we have in the past, we will expect all Lakeland parents & guardians to conduct daily symptom screening with all students attending school in-person. Symptom screeners do not need to be submitted, but rather should be used to monitor students for symptoms of COVID-19. Students who are exhibiting symptoms should be kept home from school until symptoms resolve or a negative COVID-19 test is obtained.
Request for Information Form:Parents who would like to request additional information related to a return to full in-person learning may submit an Information Request Form. The district will either contact the parent directly or incorporate the information into the Frequently Asked Questions document.
